The Integrated Care Programme for Older Persons (ICPOP) improves the life of older persons by providing access to integrated care and support that is planned around their needs and choices, supporting them to live well in their own homes and communities. 

About Nora

Hello, I am Nora and I am 85. Until recently I was living on my own and I was managing well. But after falling ill, I fell and broke my pelvic bones. I came to the hospital and had to stay there for a long time. Afterwards, the doctors decided that I had to go to a nursing home. But my care could have been better coordinated with the Integrated Care Programme for Older Persons. I could still live at home, close to my family and friends and I would get the care I need.


The Integrated Care Programme for Older Persons (ICPOP) could provide person-centred co-ordinated care across health and social care services for Nora and enable her to live at home with every support she needs.
